Job Type: 12 Month contract

Hours: 38 hours per week

 

What will you be doing as a Senior Care Support Worker

  • Supporting the service manager and field care supervisor with assessing, reviewing and risk assessing both new and current client needs ensuring they are met to a high standard to improving carers and the people they care for quality of life whilst adhering to current regulations and guidelines.
  • Working within the community of York City and surrounding areas providing Health and Social Care Support to clients and their carers via short breaks/respite at home.
  • The post holder will provide a respite service to improve the quality of life for carers by providing timely breaks for carers, preventing carer breakdown, ensure that carers are supported to remain in their caring role, and prevent unnecessary referrals to health and social care services by assessing and risk assessing both new and current clients to ensure their needs are met and safe practice is followed.
  • To conduct assessments and risk assessments for new services.
  • When required, to carry out reviews for existing services and update support plans accordingly.
  • To undertake the tasks specified in the support plan, including support with personal care and mobility.
  • To maintain accurate records as directed by the service manager and carry out administrative tasks as required.
  • To participate in supervision, appraisal and training sessions to ensure that standards are maintained.
  • Any other tasks deemed reasonable within the boundaries of the role.
