Temporary care for individuals transitioning from hospital, assisting them at home to prevent unnecessary hospital admissions.

The service assists individuals with care needs who are in the hospital and preparing for discharge.

The hospital's adult social team determines the appropriate pathway for the patient being discharged. If the patient is deemed suitable for the service, a referral is submitted. Our staff will then arrange an assessment and implement care calls as needed.

The service supports individuals for a duration of up to four weeks. During this time, staff assess clients' needs and identify those who require a long-term care package. Individuals in need of ongoing care are referred to the adult social care team, which arranges for a long-term care provider.

The aim of the service is to ensure people leaving hospital in North Northamptonshire get the support they need to continue their recovery at home and reduce the likelihood of them returning to hospital. The aim is to help people who have been in hospital to regain their independence.
