About the Role

Our Field Assessment team provide support for older adults (65+) and people with a physical disability and their carers.  We focus on helping people to keep their independence by utilising community resources and social networks that will enable the person to live a full and rewarding life,

You will be completing a range of electronic care and support plans within client’s homes.  The aim being that we deliver high quality, person centred care for our clients and are compliant with our regulators.  The role requires a high level of confidence in using technology, providing timely and effective assessments that give real time information to the wider care team.

You will have experience of working in care, and be familiar with the Care Quality Commission standards and assessment processes. This role will support our Projects & Services across all areas of quality assurance as required including initial assessments, risk assessments, mental capacity assessments and reviews of the service.

About You

The successful candidate will be passionate about supporting people in a person centred way.  You will have the ability to work on your own with good verbal and written communication

You will -

  • Have a minimum of 2 years’ experience of working in health and social care.
  • Proven experience of Care Planning and Risk Assessment within a client’s home setting.
  • Proven experience of medication handling and assessments.
  • Experience of low level conflict resolution and complaint handling.
  • Experience of lone working and diary management
  • Ability to provide high quality personal care and to recognise and implement through good care planning the individual needs of the service user.
  • A good general knowledge and application of IT systems including MS office and Outlook.
  • Ability to communicate effectively with a wide audience of stakeholders including service users their carers and families, community and hospital based health professionals.
  • Ability to travel across Nottinghamshire or a wider area if required to support the needs of the service.
